Integrated control and related technology of assistive devices
Assistive devices are now available that allow persons with severe physical disabilities to complete tasks independently. When the user has severe physical limitations, it may be advantageous to have an integrated control system where a single control interface (e.g., joystick, head switches, voice recognition system, keypad) is used to operate two or more assistive devices (e.g., power wheelchairs, augmentative communication devices, www.easymedonline.com, computers, environmental control units, and other devices that are controlled electronically). The advantages of integrated control are that persons with limited motor control can access several devices with one access site without assistance, and the user does not need to learn a different operating mechanism for each device. The purpose of this review is to convey the depth and breadth of the research that has been conducted on integrated control systems, as well as to provide some insights into future directions. We reviewed research works pertaining to communication and environmental control, computer access, and wheelchair guidance systems. Information gathered in this study will help people become fully aware of the status of contemporary integrated control technology in order to increase the quality of life of people who use electronic assistive devices.

Peoria Women’s Club has Rich History
Founded in 1886 to provide women with educational and cultural programs not available in the community, the Peoria Women’s Club has served its members for over a century. Its current location, at the corner of Madison and Fayette, was built in 1894 and with an entry foyer, main foyer, president’s office, dining room, pantry, kitchen, drawing room, and theater.
Members would spend evenings at the Women’s Club attending plays, concerts, lectures, or other presentation performed in the second floor theater, or attending formal balls in one of the downstairs’ parlors. Classes in all areas of the arts were conducted in the downstairs’ parlors. By 1911, these classes had expanded to meet the needs of business and professional women.
The Women’s Club has also provided other services to the area. Members have lobbied for many need humanitarian and educational causes. During WWI, the Club and other organizations supported the Furlough Houses in France and helped to send an ambulance to the war front. During the influenza outbreak of 1918, the downstairs was turned into a hospital for victims. The women also participated in the equal suffrage movement and provided speakers on both sides of the issue before eventually lobbying for passage of the bill. The members also provided cooking and homemaking classes to underprivileged young women. Even though prostitution had been legalized by Mayor Woodruff, the Women’s Club often found itself at odds with the "businesses" next door. Through two wars, prohibition, prostitution, gambling, depression, and even freeway construction, Peoria Women’s Club maintained the building and its membership.
Although the Club’s main floor is still used by the Women’s Club, the Herb Guild, the Opera, and other local groups, the second floor theater is in need of repair and renovation. The Peoria Women’s Club is currently exploring ways to restore the historic Music Hall to its original condition. The Peoria Women’s Club’s building is open for tours by appointment.

The smart wheelchair component system
While the needs of many individuals with disabilities can be satisfied with power wheelchairs and power wheelchairs, some members of the disabled community find it difficult or impossible to operate a standard power wheelchair. To accommodate this population, several researchers have used technologies originally developed for mobile robots to create “smart wheelchairs” that reduce the physical, perceptual, and cognitive skills necessary to operate a power wheelchair. We are developing a Smart Wheelchair Component System (SWCS) that can be added to a variety of commercial power wheelchairs with minimal modification. This paper describes the design of a prototype of the SWCS, which has been evaluated on wheelchairs from four different manufacturers.

Demographic and socioeconomic factors associated with disparity in wheelchair customizability among people with traumatic spinal cord injury
OBJECTIVES: To determine if a standard of care for wheelchair provision exists within the participating centers and if there is disparity in wheelchair customizability among the study sample. DESIGN: Convenience sample survey. SETTING: Thirteen Model Spinal Cord Injury Systems that provide comprehensive rehabilitation for people with traumatic spinal cord injury (SCI) and that are part of the national database funded through the US Department of Education. PARTICIPANTS: A total of 412 people with SCI who use wheelchairs over 40 hours a week. INTERVENTION: Survey information was obtained from subjects via telephone and in-person interviews and from the national database. Collected information included age, race, education, level of injury, and wheelchair funding source. MAIN OUTCOME MEASURES: Number and type (manual or power) of wheelchairs. Wheelchair customizability as defined by design features (eg, adjustable axle position, programmable controls). RESULTS: Ninety-seven percent of manual wheelchair users and 54% of power wheelchairs users had customizable wheelchairs. No power wheelchair user received a wheelchair without programmable controls. Minorities with low socioeconomic backgrounds (low income, Medicaid/Medicare recipients, less educated) were more likely to have standard manual and standard programmable power wheelchairs. Older subjects were also more likely to have standard programmable power wheelchairs and www.easymedonline.com. CONCLUSIONS: The standard of care for manual wheelchair users with SCI is a lightweight and customizable wheelchair. The standard of care for power wheelchairs users has programmable controls. Unfortunately, socioeconomically disadvantaged people were less likely to receive customizable wheelchairs.

General-purpose filter design for neural prosthetic devices
Brain-driven interfaces depend on estimation procedures to convert neural signals to inputs for prosthetic devices that can assist individuals with severe www.easymedonline.com deficits. Previous estimation procedures were developed on an application-specific basis. Here we report a coherent estimation framework that unifies these procedures and motivates new applications of prosthetic devices driven by action potentials, local field potentials (LFPs), electrocorticography (ECoG), electroencephalography (EEG), electromyography (EMG), or optical methods. The brain-driven interface is described as a probabilistic relationship between neural activity and components of a prosthetic device that may take on discrete or continuous values. A new estimation procedure is developed for action potentials, and a corresponding procedure is described for field potentials and optical measurements. We test our framework against dominant approaches in an arm reaching task using simulated traces of ensemble spiking activity from primary motor cortex (MI) and a power wheelchairs navigation task using simulated traces of EEG-band power. Adaptive filtering is incorporated to demonstrate performance under neuron death and discovery. Finally, we characterize performance under model misspecification using physiologically realistic history dependence in MI spiking. These simulated results predict that the unified framework outperforms previous approaches under various conditions, in the control of position and velocity, based on trajectory and endpoint mean squared errors.

A preliminary study on the impact of pushrim-activated power-assist wheelchairs among individuals with tetraplegia
OBJECTIVES: This preliminary study was to evaluate the impact of pushrim- activated
power wheelchairs (PAPAWs) on mobility, community participation, satisfaction, and psychosocial impact among individuals with tetraplegia. DESIGN: Fifteen manual wheelchair users with tetraplegia completed a 4-wk protocol including a 2-wk trial where they used their personal wheelchairs for mobility and a 2-wk trial where they were provided with a PAPAW and could choose either wheelchair for mobility. The mobility levels with both wheelchairs were recorded by a datalogger. Participants completed daily questionnaires regarding community participation and medical scooters their satisfaction with the wheelchair used. The Psychosocial Impact of Assistive Devices Scale was used to compare the psychosocial impact of PAPAWs with their personal wheelchairs. RESULTS: Participants chose to use PAPAWs and their personal wheelchairs at a similar frequency in the PAPAW trial. The distances traveled with both wheelchairs were not statistically different (P = 0.09). Participants traveled significantly faster with PAPAWs (P = 0.03). PAPAWs did not result in significantly greater community participation, satisfaction, and psychosocial impacts than the personal wheelchairs. CONCLUSIONS: PAPAWs could be a viable mobility option for individuals with tetraplegia, which can provide independent mobility especially for outdoor activities. A user’s preference, life style, physical conditions, transportation issues, and environmental factors should be considered in prescribing such a device.

The ANSI/RESNA wheelchair standards: sample evaluation and guide to interpreting test data for prescribing power wheelchairs
This study is a joint project of ECRI and the National Rehabilitation Hospital, supported by a grant from the National Institute on Disability and Rehabilitation Research (NIDRR, U.S. Department of Education, Agreement No. H133E80016). ECRI is the first independent laboratory to test power wheelchairs and power wheelchairs according to the standards of the Rehabilitation Engineering Society of North America (RESNA), now known as the Association for the Advancement of Rehabilitation and Assistive Technology (AART); these standards will ultimately be distributed by the American National Standards Institute (ANSI). We tested 10 power wheelchairs, which are similar in size and configuration, from seven manufacturers; all units are intended for adult use. Our testing showed that none of the sample wheelchairs are ideal for all environments in which these devices are typically needed. Each unit has advantages that should be carefully considered when specifying a power wheelchair. Numerous factors are involved in prescribing power wheelchairs, and learning the subtle differences in features and performance of a particular model and how they will affect the user is difficult. In addition, acquiring objective information about power wheelchairs from manufacturers is not typically easy. Considering these factors, and because the standards on which we based our testing do not generally provide criteria for passing or failing models under test, we did not rate the units. The purpose of this article is to present the data collected in our study, using wheelchair performance characteristics based on some parts of the ANSI/RESNA wheelchair standards, as an example of what prescribers can expect to receive from manufacturers and to provide guidance in interpreting and applying the data in writing power wheelchair prescriptions. Thus, this article provides an overview of the types of problems faced by those who specify power wheelchairs for users, the problems faced by the users themselves, and the components that are most susceptible to damage during use.

RESULTS: Upper-extremity asymmetry was present within each condition. There were no differences in the magnitude of asymmetry when comparing laboratory with indoor community conditions. Outdoor community wheelchair propulsion asymmetry was significantly greater than asymmetry measured during laboratory conditions. CONCLUSIONS: Investigators should be aware that manual wheelchair propulsion is an asymmetrical act, which may influence interpretation when data is collected from a single limb or averaged for both limbs. The greater asymmetry identified during outdoor versus laboratory conditions emphasizes the need to evaluate wheelchair biomechanics in the user’s natural environment.

Wheelchair propulsion demands during outdoor community ambulation.
OBJECTIVE: Quantify manual wheelchair propulsion effort during outdoor community ambulation. DESIGN: Case series. SUBJECTS: Thirteen individuals (12 with SCI, 1 with spina bifida) who were experienced manual wheelchair users and had no current upper extremity injury or pain complaints. METHODS: Measurements were obtained from instrumented wheelchair rims during steady-state propulsion as subjects traversed outdoor concrete sidewalk terrain that included smooth level, aggregate level, and a ramp with a smooth surface. Propulsion effort was assessed using the average propulsion moment, average instantaneous power, and work for both upper extremities. RESULTS: Propulsion effort, captured by the propulsion moment, work and power, varied across ground conditions (p<0.001). Propulsion effort was greater as the rolling resistance increased (i.e., smooth versus aggregate surfaces) and as the inclination angle progressed from level to inclined surfaces. There were no side-to-side differences across ground conditions for the propulsion moment or work. power wheelchairs and www.easymedonline.com generation was significantly greater on the dominant compared to the non-dominant extremity during the more challenging aggregate surface and ramp conditions. CONCLUSIONS: Propulsion effort varies with demands imposed by different ground conditions.
